Pasco County Driver License Document Checklist

1. Identification - Bring ONE of the following:
- Certified U.S. Birth Certificate (Issued by government agency; hospital birth certificate not accepted).
- Valid U.S. Passport.
- Certification of Naturalization.
- Consular Proof of Birth Abroad.
- Alien Registration Receipt Card (Green Card, Form I-551).
2. Social Security Number - Bring ONE of the following:
- Social Security Card.
- W-2 Form (preprinted, cannot be handwritten).
- Paycheck showing complete number.
- IRS Form 1099-MISC.
3. Residential Address - Bring TWO of the following:

- Deed, Mortgage, Payment Booklet or Rental Agreement. *Must be dated within the last 60 days.

4. Name Change - if applicable:

Apply for a First-Time Florida Driver License (also known as a Learner Permit) and the applicant has never been licensed by any state or county
The following is required to apply for a Learner Permit:
For applicants 17 years or younger:
- United States Birth Certificate OR valid United States Passport. A birth certificate must be issued by a government agency. Hospital birth certificates cannot be accepted.
- Social Security Card or one of the following documents showing your full social security number: W-2 form, paycheck stub, or Form 1099. The name assigned to the social security number must match the name that will appear on the Florida Driver License. If you have recently changed your name, update your records with the Social Security Administration before you apply for your license.
- ATTENTION: Effective August 1, 2025 Learner's Permit applicants must complete the Driver Education Traffic Safety Course. Click Here for Courses - *This requirement does NOT apply for applicants who completed their TLSAE (Traffic Law Substance Abuse Education Course) prior to August 1, 2025*
- Parental Consent Form either signed and notarized in advance, or signed in the presence of our examiner. Person signing the consent form must be legal guardian. If other than parent, such as grandparent, proof of guardianship must be provided.
- Vision Test
- 45-Question Road Rules Test and 5-Question Road Sign Test, which must begin no later than one hour before office closing time. Official Florida Driver License Handbook. For Practice Tests: Click here. Test must be administered in English ONLY as required by State DMV.
- If using an On-Line Third Party Tester/Administrator, a Parent/Guardian On-Line Test Proctoring Form must be signed in the presence of a driver license examiner by the person who proctored the test; or the proctor's signature must be notarized if the parent or guardian proctor will not be present. Also, per Florida Statute 322.56(3) a customer who takes and passes the Class E Knowledge or Skills Exam through an authorized Third Party Administrator may be randomly selected for a mandatory re-test without prior notice. A failed re-test will require a re-exam fee to be collected at the next exam attempt. Knowledge and Driving Skills Exam Providers-Third Party.
- Proof of residential address requires two of the following (dated within the past 60 days) reflecting your name and address: deed, mortgage statement, lease agreement, voter registration, vehicle registration, W2 form, 1099 form, or a homeowners, automobile, or life insurance policy. - If the documentation submitted reflects any name change, an original or certified copy of your Marriage License, Divorce Decree or Court Order, must be submitted. In some instances, more than one marriage license, divorce decree or court order document may be required to show the chain of name changes. Photocopies cannot be accepted. Marriage license must be from a governmental agency. Church issued licenses cannot be accepted.

NOTICE: All exams must be administered in English as required by the State DMV.
Attention: Effective August 1, 2025, for applicants 18 years or older making application for a Florida Driver License you must provide proof of completion of a traffic law substance abuse education course. Click the link for a list of Driver Education Licensing Assistance Program Providers.
The following is required to transfer a Florida Learner's License to a Florida Driver License:
- You must be at least 16 years old and have held a Learner's License for at least one year without any traffic convictions.
- Provide a Florida Learner's License. Pay tax collector fee of $6.25. Additional fees may be required.
- If 17 years of age or younger, Certification of Driving Experience, ONLY required if a responsible adult over the age of 21 is not present for the transaction, either signed and notarized in advance, or signed in the presence of our examiner, certifying that the applicant has had a minimum of 50 hours of behind-the-wheel driving experience, 10 hours of which was night driving.
- Per Florida Statute 322.56(3) a customer who takes and passes the Class E Knowledge or Skills Exam through an authorized Third-Party Administrator may be randomly selected for a mandatory re-test without prior notice. A failed re-test will require a re-exam fee be collected at the next exam attempt. Knowledge and Driving Skills Exam Providers-Third Party.
- Driving test: Click below to schedule an appointment. Must arrive no later than 15 minutes from scheduled appointment time. All Driving tests will be administered in English. The driving tester/examiner's decision about your performance is final in all cases. If an extended driving test is required, you will need to schedule your appointment in two time slots back to back. Walk in road tests are available for Pasco residents ONLY, during the hours of 8:30-4pm. These are first come, first serve. Saturdays are Pasco Day and are reserved for Pasco County Residents Only. No Driver License Services are offered after 11:00am on Saturdays.
- Driving tests will NOT be given if the inspection of the driving test vehicle finds:
- The horn, rearview mirror, directional signals on trucks, steering wheel, footbrake, tires, brake lights, or taillights are defective or inoperable.
- No windshield wipers on the driver’s side of the vehicle when raining.
- No operable headlights when visibility is reduced to 1,000’ or less due to rain, fog, smoke or other dangers.
- Glass hinders visibility.
- The vehicle is not equipped with doors. Front doors do not open from the inside and outside.
- Vehicle does not have stationary seats.
- Vehicle does not permit the driver to give proper hand signals due to a blocked or inoperative window when hand signals are required.
- Jeep type vehicles without framed canvas or metal doors held by hinges and door latch. The doors must be secured to contain the driver/passenger if they are thrown against the door.
- Vehicle does not meet the bumper height requirements outlined in section 316.251, Florida Statutes.
- Vehicle is a low speed vehicle.
- No valid license plate, decal, registration or proof of insurance.
- Tires that are bald, excessively worn or cord showing.
- Vehicle air conditioning inoperable & windows inoperable to allow for fresh air during testing.
- Any reasonable circumstance making the vehicle unfit for test i.e. cleanliness, disrepair, etc.
- Any change in address or name will require additional documentation.

You will need to have the following documents with you and ready to provide to tester upon arrival:
- Valid vehicle registration and current Florida insurance
- Have already read the Driving Skill Road Test Checklist.
- Completed Certification of Driving Experience, ONLY required if a responsible adult over the age of 21 is not present for the transaction.
- Working air conditioning. Road test will be conducted with the windows down.
If any documents are missing or incorrect, we will be unable to administer the test and you will need to reschedule at a later date.

Renew a Florida Driver License or Florida Identification Card
The following documentation is required to renew your Florida Driver License or Florida Identification card in our office, unless you are already REAL ID Compliant with a Gold Star in the top right corner of your current Florida Driver License or Florida Identification card. If you are changing your address during your renewal, two proofs of your new residential address will be required.For a complete list of items, click Proof of Residential Address.
A photo taken in office is required when issuing a Driver License, Replacement License, FL Identification Card or Renewing a FL Identification Card
A vision test will be required upon an original or renewal of a Driver License. To acquire a driver’s license in the State of Florida, a customer must pass a vision test to determine if they meet the mandatory minimum vision standards. This testing will be done in the office during your renewal transaction. For individuals not meeting the minimum vision standards a FLHSMV Report of Eye Exam Form 72010 must be completed by a licensed ophthalmologist, optometrist, or physician.
Drivers over 80 years of age must submit FLHSMV Vision Examination Form 72119 when renewing their driver license OR take and pass the vision test at our office.
Effective November 1, 2009, a Florida Driver License and Florida Identification Card may no longer be issued to the same person.

Renewing Florida Driver License:
Driver License renewal forms are mailed directly from the Florida Division of Driver Licenses in Tallahassee.
The renewal form will indicate whether you may renew by mail or online or if you must renew in person.
- At this time, State Policy does not permit Tax Collectors to renew Driver Licenses by mail. You may mail your renewal card and check to the Division's Tallahassee address provided on the renewal card, OR you may renew over the Internet at the web address provided on the renewal card, or through MyDMV Portal
- Further information regarding renewal by mail may be obtained from the Florida Division of Driver Licenses. Contact Customer Service at (850) 617-2000, Monday thru Friday.
- If required, you may renew in person at our office. Florida law requires an additional $6.25 service charge for renewal at any Tax Collector Office.
Renewing Florida Identification Card:
The Florida Division of Driver Licenses does not mail renewal notices for Identification Cards. Identification Cards can be renewed over Internet at MyDMV Portal or at our office. Identification Cards cannot be renewed by mail. The expiration date will be found on the card.

The federal USA PATRIOT Act requires Commercial Driver License (CDL) holders to pass a national background check administered by the Department of Homeland Security, Transportation Security Administration (TSA) before their licenses may be endorsed for transport of hazardous materials. New, current, or expired HAZMAT endorsement applicants must be fingerprinted, pass the required national-level background check, and present proof of residency prior to receiving the hazardous materials endorsement.
If you receive information from the Transportation Security Administration (TSA) that you did not pass the background check, you may apply for an appeal, or in some cases apply for a waiver. The appeal must be made through TSA. You may contact TSA HAZMAT at (855) 347-8371 (Weekdays from 8 am to 10 pm ET).
NEW Entry-Level Driver Training Requirements for CDL Applicants
The new Entry-Level Driver Training (ELDT) regulations require that all entry-level drivers of commercial motor vehicles (CMVs) receive training from a provider listed on FMCSA’s Training Provider Registry.
Training must be completed before taking the hazardous materials knowledge test (H) unless a hazardous materials endorsement (H) was obtained on a CDL prior to February 7. 2022.

Fingerprint and Application Process:
- The written Hazmat exam is required to be taken for all original Hazmat and all renewal Hazmat endorsed license applications.
- Fingerprinting must be done in a Driver License office at the same time the application for a Hazmat endorsement is made. The Tax Collector offers Hazmat services at our New Port Richey, West Pasco Government Center location ONLY 8:30 to 4:00.
- An application form must be completed at the time of fingerprinting to be sent to TSA.
- An application fee of $91 will be charged to cover costs of the background check process, in addition to the standard driver license issuance fee for the Commercial Driver License and for each endorsement.
- New residents of Florida requesting the Hazmat endorsement be added to their Florida Commercial Driver License must retake the written test AND redo the background check and finger printing regardless of when they processed it previously. Fees for the background check and fingerprinting must also be paid again.

Pasco Tax Collector Mobile Unit (PTCMU)
Pasco Tax Collector Mobile Unit (PTCMU) is available for remote processing of identification card and driver license services to Florida residents who reside in a nursing home or assisted living facility located in Pasco County.
The Pasco Tax Collector Mobile Unit (PTCMU) grants those with the inability to travel to one of our offices the opportunity to renew or replace their Florida Driver License or Florida Identification Card. The entire process takes place onsite, with the driver license/identification card being delivered the same day.

Motorcycle Driver License Information
Motorcycle Driver License Information
Special licensing is required for drivers of motorcycles, which will appear as an endorsement on the face of their Florida Driver License; or as a restricted "Motorcycle Only" Florida Driver License.
Florida law requires all motorcycle drivers to complete Motorcycle Safety Education Course from a licensed motorcycle driving school. The course consists of written and driving tests. For a list of approved Motorcycle Safety Education Course providers near you call 1-850-488-3286, or visit the state's website at https://www.flhsmv.gov/driver-licenses-id-cards/motorcycle-rider-education-endorsements/safety-course-locations/. The cost for the course varies by location and course provider.
Upon completion of the Motorcycle Safety Education Course, the state approved provider will issue a Course Completion Certificate. The Course Completion Certificate must be submitted to our office to obtain a new Driver License with a Motorcycle Endorsement.

If your Florida Driver License or Identification Card has been lost or stolen, you may obtain a replacement at our office.
Police Report indicating driver’s license or identification card stolen will be required for a no-fee replacement for Pasco County Residents. Any changes to name, address, or other information will require a fee.

A replacement Florida Driver License or Identification Card with a new address and photo may be obtained at our office. If you are REAL ID compliant with the Gold Star you will only need two proofs of your new residential address, if you're doing an address change.

You may complete and mail a Division of Driver License Mail-In Address Change Form to Tallahassee with the required fee for a Driver License or an Identification Card. Either a new license or decal reflecting your new address will be provided

A name change requires the issuance of a new Driver License or Identification Card.
If your Social Security card also needs to be updated with the Social Security Administration you must do that before coming to our office and wait 24-48 hours for the Social Security Administration to update their records. We will then be able to process your name change in our office.

Honorably discharged veterans have the opportunity to add a special Veteran "V" designation to their Florida Driver License or Florida Identification Card.

Non-US Citizen Driver Licenses

New Drivers License/ID Card Law
What is this new Driver License / Identification Card law about?
Additional proof of identity documents are now required by the "Real ID" Act, passed by the United States Congress in 2005 as a result of the terrorist acts of September 11, 2001.
Florida implemented the law January 1, 2010.
The new "secure identity" driver license is identified by a gold star in the upper right corner.
How do I know what documents to bring?

Can I bring copies of the required documents?
No. For office visits, original documents (some must be certified by a governmental agency) must be submitted.
Can I postpone complying with the new law?
Most Florida residents are allowed to renew their driver license by mail or by Internet once and receive a "standard" Driver License. The next renewal must be in person, at which time the required documents must be provided.
However, the "standard" Driver License is subject to future restrictions.
What payment options do you offer?
Mail: Pay by check.
Internet: Pay online by VISA, MasterCard, Discover, American Express; or E-Check.
In person: Pay at any of our four offices by cash, check, VISA, MasterCard, Discover or American Express.
A vendor fee is charged by our third party provider for credit / debit card and E-Check transactions. Our office does not charge, collect or retain any portion of this fee.
Why am I charged a fee to use my credit or debit card at your office?
Credit card vendors charge private businesses and governmental agencies a fee on each transaction processed with a credit or debit card.
While private businesses can increase the price of their merchandise to recover this additional cost, governmental agencies cannot. Consequently, we must reflect the vendor fee separately.
The credit / debit fee is authorized by Florida Statute 215.322 and is paid to the credit card vendor; none of it is retained by our office.
CDL Testing and Licensing
NOTICE: We are currently suspending service for Non-US Citizens who are Out of County Residents. Please contact your home county for service.
All five Pasco Tax Collector’s Offices also are able to issue and renew CDL Licenses. When obtaining or renewing a CDL please remember to have a CDL Medical Certificate in your possession when visiting our office.
Effective October 7, 2024, the Florida Department of Highway Safety and Motor Vehicles requires customers applying for an Original CDL or CLP must present one of the documents listed below to establish Florida residency. If the document presented is included on List A or B (below) then it will be used as both address and residency proof. Otherwise, this is an additional requirement.
- A current Florida Class E Driver License or Florida Identification card held for a period of more than 6 consecutive months.
- A lease agreement, in the customer’s name, that has been in place for a period of more than 6 consecutive months.
- A current residential mortgage, in the customer’s name, signed more than 6 months ago.
- A current residential deed, in the customer’s name, signed more than 6 months ago.
- A Florida Voter Registration Card.
- A statement of domicile pursuant to section 222.17.F.S
- A homestead tax exemption filing on property in this state
Effective June 1, 2022 the Florida Department of Highway Safety and Motor Vehicles revised their policy on Residential Address for Issuing Credentials for all Commercial License Permit (CLP) or Commercial Driver License (CDL) applicants. These individuals must present two proofs of Florida residential address in their name.
Applicants must present one proof from List A and one proof from List B below or two from List A.
LIST A for CLP / CDL - item presented must be in the applicant's name.
- A recent lease or rental agreement for the residence with a term of 6 months or greater (must be dated within the last 6 months
- Current Florida voter registration card
- Florida vehicle/vessel registration or title
- Current homeowner's insurance policy or bill
- Current electricity or water bill
- Recent cellular or landline telephone bill (must be dated within the last 30 to 60 days)
- Recent internet service provider bill (must be dated within the last 30 to 60 days)
- Latest property tax bill
- Mortgage deed for property in Florida
LIST B for CLP / CDL - item presented must be in the applicant's name.
- Latest W-2 form or 1099 form (must be dated within the last 6 months)
- Recent bank statement (must be dated within the last 60 days)
- Recent credit card statement (must be dated within the last 60 days)
- Latest military orders
- Recent pay stub (must be dated within the last 60 days)
- Recent official government documents (must be dated within the last 60 days)
CLP or CDL applicants unable to provide a proof of residential address from List A, must provide a Declaration of Domicile filed with the Clerk of the Circuit Court pursuant to Florida Statues section 222.17 AND one proof of residential address document from List B.
A Certification of Address form (HSMV-71120) can no longer be accepted. CLP and CDL applicants upgrading from a Class E or ID card must resubmit acceptable proof of residential address.

- Non-Pasco County Residents must arrive in our office by 4pm to be served Monday through Friday
- Saturday -- 8:30 AM - 12:00 PM (PASCO RESIDENTS ONLY - Wesley Chapel and Gulf Harbors Offices ONLY). No Driver License Services are offered after 11:30am
- Our office does not administer Commercial Motor Vehicle Skill Tests. These tests are provided at specific State of Florida or state contracted CDL testing sites. A listing of sites that administer CDL testing is available at www.flhsmv.gov.
- Florida law requires an additional $6.25 service charge for Driver License Transactions processed at any Tax Collector office.
